Infants with torticollis, plagiocephaly, body tension, and gross motor delays (rolling, sitting, crawling, standing, walking), as well as premature infants and NICU graduates. Toddlers with delayed walking, asymmetrical movement, decreased coordination, difficulty navigating stairs, transitioning on/off the floor, or jumping. Infants and young children with special needs (0–5 years) including Cerebral Palsy, Spina Bifida, Down Syndrome, genetic and neurological conditions, brain injury, spinal cord injury, and hypotonia.

In Utah, a referral is not required for physical therapy evaluation or treatment. Some insurance plans may require a referral for reimbursement if you plan to self-submit. Checking with your insurance provider is recommended.

It depends on age, tolerance, and condition severity. A recommended plan of care and prognosis will be provided at the evaluation. Earlier intervention can reduce total treatment duration for some conditions (e.g., torticollis). Parents are also taught home activities to support progress between sessions.

Most appointments are scheduled for one hour. Sessions may range from 45–60 minutes depending on the child's tolerance and readiness. Part of the session includes parent education.

The Village SLC is an out-of-network, cash-based practice and does not bill insurance directly. Payment is due at the time of service. A superbill with medical codes can be provided for self-submission, but reimbursement depends on your individual plan and is not guaranteed. The Village SLC is not a Medicaid provider. Wellness visits and any travel fees are not reimbursable. HSA and FSA cards are accepted.

The Village SLC serves families throughout the Salt Lake Valley (Bountiful to Lehi). A $40 travel fee applies to surrounding areas including Summit Park, Park City, Heber City, Tooele, American Fork, Pleasant Grove, Lindon, Orem, Provo, Saratoga Springs, Eagle Mountain, Centerville, Farmington, Kaysville, Layton, and Ogden. If you live outside this range, please reach out — services may still be available with an adjusted travel fee.
